The most common approach to complete male to female reassignment surgery is the penile inversion. In this technique the skin of the penis is turned inside out to line the walls of the new vagina.
In some cases, extra skin is required to give more depth; this is usually taken from the lower abdomen or the scrotal sac.
As part of the penile inversion, a small section of the head of the penis is used to create a new clitoris. The urethra is shortened and relocated as part of the vaginoplasty.
The inner labia are created from excess skin of the penis and the outer labia are created from excess skin from the testicles.
